The Mississauga Halton Central Intake Program is a regional referral and coordination centre established to streamline access to care in the Mississauga Halton Region. The program is funded by the Mississauga Halton Local Health Integration Network (LHIN) and is managed and hosted by Halton Healthcare.
Launched in 2013, the Mississauga Halton Central Intake Program serves as a single access point for referrals to:
- Diabetes Education Programs
- Addiction & Mental Health Services (one-Link)
- Foot Care Services
- Musculoskeletal Care
- Hip & Knee Rapid Access Clinic
- Low Back Pain Rapid Access Clinic
The Central Intake Program is the single point of access to coordinate referrals to the right care, the right place, at the right time with integrated communication between all parties.
Our team consists of health care professionals and administrative staff who work with health care providers, service providers and patients to access services easily and in a timely manner, supporting consistency and best practices among services.
