Mississauga Halton Central Intake eReferral provides secure access to submit referrals online for Diabetes Education, Addictions & Mental Health and Hip & Knee and Lower Back Rapid Access Clinics.

The eReferral system offers health care providers in hospitals, long-term care homes, community care access centres and others in the health sector a convenient one-stop source for consistent, timely and trusted health information. Referrals can be saved as a draft and are checked for completeness prior to submission.

When accessing eReferral the application will allow you to:

  • Upload/scan & attach supporting documents to requests
  • See the current status of each referral
  • Access updates in real time
  • Submit referrals for multiple chronic conditions

The Mississauga Halton Central Intake Program works with the Novari Health eRequest platform to implement an electronic eReferral solution with central intake capability across the region.

Registration Requirements:

  • Must be a Health Care Provider (not open to patients for self referring)
  • The provider must complete a multi-document user agreement
  • Email address

Technical Support – Helpful Tips

  • Verify that your browser is up to date
  • Clear browser ‘cache’
    •  Hit Ctrl F5 to clear your computer cache
      (The cache (pronounced “cash”) is a space in your computer’s hard drive and in RAM memory where your browser saves copies of previously visited web pages. Your browser uses the cache like a short term memory)
    • Close all open windows and re-launch the browser
  • Clear computer cookies
