To support equitable access to a high quality continuum for musculoskeletal care, Ontario Health in partnership with Central Region Hospitals operates the Rapid Access Clinic (RAC) for patients with moderate to severe Osteoarthritis.

Referral Process

The RAC for Shoulder Pain helps patients 18 and older living with shoulder pain and provides timely access to an assessment by an Advanced Practice Practitioner (Regulated Healthcare Professional with advanced training in orthopaedic care) within 2-4 weeks.
RAC Shoulder referrals are currently being accepted at Halton Healthcare & Oak Valley Health within the OH Central Region.

Mandatory Referral Information

Patient Information:  Last Name, First Name, DOB, OHIP, Address, Phone Number
Referring Provider: Name, Address, Phone number, Fax number, Billing number, Signature
Primary Problem: Indication of which side (left, right or bilateral) and joint (shoulder)
Diagnosis: At least one must be selected
Diagnostic Imaging Requirements: X-Ray report on the corresponding joint completed within the last 9 months, as well as an ultrasound and/OR MRI for soft tissue diagnosis completed within the last 9 months
Cumulative Patient Profile (CPP): not mandatory but Central Intake is to request for all referrals, document attempt in the Central Intake banner, if referral is otherwise complete route to RAC.  Once CIP receives CPP after routing they are to attach to referral. (Mandatory for Surgeons Offices)

INCLUSION CRITERIA

EXCLUSION CRITERIA

Patients MUST be 18 years of age or older with shoulder pain
Diagnosis of Rotator cuff disorder or tear, Inflammatory arthritis, Osteoarthritis, Instability/labral tear, AC joint, Impingement syndrome, Adhesive capsulitis
Prior Arthroplasty with peri-prosthetic fracture infection, recurrent dislocation, revisions, Acute tendon rupture
Urgent Referrals Characterized by: An immediate threat to role and independence, potentially requiring hospitalization within 4 weeks
Malignancy
Active WSIB or MVA Claims
Out of Province
