To support equitable access to a high quality continuum for musculoskeletal care, Ontario Health in partnership with Central Region Hospitals operates the Rapid Access Clinic (RAC) for patients with moderate to severe Osteoarthritis.

Referral Criteria

Required Imaging: Guidelines to support you in determining the appropriate imaging for hip and knee osteoarthritis.

Mandatory Referral Information

Patient Information:  Last Name, First Name, DOB, OHIP, Address, Phone Number
Primary Problem: Indication of which side (left, right or bilateral) and joint (hip, knee and/or both)
Diagnosis: At least one must be selected
Referring Provider: Name, Address, Phone number, Fax number, Billing number, Signature
Cumulative Patient Profile (CPP) – not mandatory but Central Intake is to request for all referrals, document attempt in the Central Intake banner, if referral is otherwise complete route to RAC.  Once CIP receives CPP after routing they are to attach to referral. (Mandatory for Surgeons Offices)

EXCLUSION CRITERIA

Patient MUST Be 18 years of age or older
Moderate to Severe Hip and Knee Arthritis
Urgent Referrals Characterized by: Prior Arthroplasty with peri-prosthetic fracture infection, recurrent dislocation
Urgent Referrals Characterized by: An immediate threat to role and independence, potentially requiring hospitalization within 4 weeks
Mild Osteoarthritis
Out of Province
