Diabetes education programs are designed to empower individuals living with diabetes and healthcare professionals with the knowledge and skills needed to manage the condition effectively.

Referral

The patient presents to their primary care provider, who makes a referral to central intake.

Appointment

The patient will be routed to a program closest to home and the program will contact you to schedule your appointment

Assessment and Education

The patient receives an assessment and education from a specially trained and regulated health care professional.

Interested in attending a Diabetes Education Program?

Contact Central Intake to start the referral process. Once the self-referral has been initiated, your family doctor or other health care provider will be contacted to complete the process and the referral will be sent to the diabetes program. The Diabetes Education Program will contact you directly to schedule an appointment once they receive your referral.
